IF YOU ARE:
You can receive dental treatment in our office via the United Concordia TDP Insurance Program. This plan operates like any other insurance policy. There is no deductible and $1,200 benefits available from Feb. 1 through Jan. 31 of each year. As a service to you, we take care of all Insurance Submissions and follow-up. As with most insurances, this plan does not pay 100% of your dental care. |

If you are an ACTIVE DUTY or NATIONAL GUARD service member, you must receive dental treatment from an IN-NETWORK dental provider to obtain any benefits from United Concordia ADDP. We have elected to NOT be an In-Network Dental Provider for this program. We feel the level of care offered to Service Members under the current ADDP coverage is not the standard they deserve. If you would like to receive dental care in our office, we will be happy to take care of you. However, be aware you will have no dental coverage through United Concordia ADDP |
